individuals, ensuring they receive the appropriate support and services tailored to their needs.

Key Responsibilities

• Coordinate and monitor care plans across health and social care services.
• Act as a key point of contact for service users, families, and professionals.
• Liaise with multidisciplinary teams to ensure continuity and quality of care.
• Maintain accurate and up-to-date records, including assessments and reviews.
• Support service users in accessing appropriate services and advocating for their needs.
• Ensure care delivery complies with safeguarding, confidentiality, and regulatory standards.
