Care Coordinator

Start Date: ImmediatePay Range: £20- £35 per hour
We are looking for a proactive and compassionate Care Coordinator to join our team. This role involves organising and overseeing care plans for
individuals, ensuring they receive the appropriate support and services tailored to their needs.
Key Responsibilities
- Coordinate and monitor care plans across health and social care services.
- Act as a key point of contact for service users, families, and professionals.
- Liaise with multidisciplinary teams to ensure continuity and quality of care.
- Maintain accurate and up-to-date records, including assessments and reviews.
- Support service users in accessing appropriate services and advocating for their needs.
- Ensure care delivery complies with safeguarding, confidentiality, and regulatory standards.
