As a mental health nurse, you’ll provide support to people living with various mental health conditions. This can involve helping the patient to recover from their illness or to come to terms with it in order to lead a positive life.

Responsibilities

The post holder will:-

• Work within an established, integrated, multidisciplinary team, focusing upon individuals with severe and enduring mental health problems.

• Undertake responsibility for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs as defined within Parts 2 and 3 of The Mental Health (Wales) Measure 2010.

• Within their role as Care Co-ordinator will facilitate the promotion and maintain a high standard of nursing care adhering to current legislation, professional and corporate guidelines.

• Work within Powys Teaching Health Board policies and procedures.

• As Care Co-ordinator, will be responsible for undertaking and documenting on the Mental health Measure paperwork, evidence-based assessments of client’s physical, psychological, spiritual and social needs, taking-into-account and respecting ethnicity, for a defined caseload.

• Assess and manage the care of clients with complex risk behaviours ensuring that a comprehensive risk assessment is completed and documented, for all actual or potential areas of risk, i.e. harm to self or others, neglect, exploitation and vulnerability.
