RMN Perinatal Practitioner - Band 6 - Aylesbury

The primary role of the Perinatal Practitioner will be to undertake comprehensive mental health assessments, manage a complex caseload of service users, provide practical advice and support including family work, psychological interventions and overseeing any safeguarding issues –adult and child protection. As well as liaising with other services working with our service users during the perinatal period. Nurse Responsibilities:

  •  The post holder will be responsible for the clinical triage of service users who access Mental Health Services during pregnancy and in the first-year post-partum and signposting on to the most appropriate service which may include follow up with the perinatal mental health team.
  • Conduct social and health assessments involving assessment of risk within the support framework of the perinatal mental health team and in a location
    acceptable to the service user usually within the service user’s home, using the perinatal assessment document.
  • The post holder will ensure that each assessment is conducted collaboratively in consultation/agreement with the service user and their carer, where available, and the assessment will be informed by the wider multidisciplinary team involved with the woman during the perinatal period, including, but not exclusively midwifery and health visiting.
  • Provide relevant advice and guidance to service users and callers including signposting out of services, transferring to IAPT and close liaison with GP’s and other agencies.
  • Provide interventions in the community as part of the perinatal service where this is the pathway of choice.
  • Provide advice and support in respect of service users open to Adult Mental Health Teams (AMHTs) who fall within the perinatal remit.
  • Provide advice or guidance in respect of criteria for admission to MBU.
